The Indiana Occupational Safety and Health Administration has cited Alcoa Warrick Operations with a serious safety violation and fined the company $5,000 in connection with a fatal accident at the plant in March.
An order signed by James W. Dolder, division director, said the company did not provide machine guarding to prevent employees from the hazards of moving machine parts when employee Dawn S. Ewing, 32, of rural Perry County, was killed.
Mike Belwood, a spokesman for Warrick Operations, said the company intended to pay the fine and had sent a letter to Indiana OSHA saying that it will correct the problems.
A serious violation lies midway on the scale of violations that Indiana OSHA might issue, said John Jones, deputy director. It indicates a more life-threatening situation than a non-serious or de minimus violation. It is considered less than a repeat violation or a willful violation, Jones said.
On March 5, Ms. Ewing was one of a crew of four working on a machine that cuts large coils of aluminum to specific widths when she became trapped between an aluminum coil and a coil car, which carries coils into position on the line. Company rescue officials on the scene within three minutes of the accident were unable to revive Ms. Ewing, who was pronounced dead at the scene.
Included in the order was a notice that, during an inspection of the plant on March 9, some machine guarding was missing at the end of a coil coating line in the finishing department.
