“The Case of Jeff: Pedophile in Institution” – Ch. 1
Overview of the Case
The case involves a 35-year-old male named Jeff in prison. He has been in prison for close to five years, and he is nearing his release. He is a pedophile and has been in treatment showing progress in the treatment. There are some inmates who do not want him released. This is because he has been offering sexual relationships in exchange for protection in prison. His pedophile character is one thing the gang knows and hence using as leverage for his compliance to their sexual advances. Jeff is now worried about his sexual orientation. He is also worried that there may be a possibility he has contracted HIV/AIDS. He, however, does not want to get tested until he gets out of prison. He also tries to hide his fears from the other inmates. This is because he easily gets sick and often feels fatigued. During his treatment, he has never mentioned the sexual experiences he has had to any of the professionals. It seems as though his sexuality is unstable at the moment that it was before. There is the fear of relapse once he is released from prison and that is one of his worries.
The Concept of Helping Jeff
In an attempt to help Jeff as a case manager, there are key concepts taken into consideration. The first one is the concept of confidentiality. This issue must remain between the client Jeff and the case manager. That is what Jeff hopes for, and that is the only way he can have faith in the therapy process. Mentioning any of such information has the risk of worsening the situation he is in which may result in loss of faith in the process rendering the program ineffective. There is also the concept of comfort that is necessary. The idea of all is going to be alright needs to be fed into his mind. Making him understand that it was ok to do what he had to do to survive is ideal for this situation. This involves making Jeff understand that there are others who had been in the same situation and did the same things but they managed to recover so he can recover too. There is also the needs to convince Jeff that there is the need to let the coordinator know about his problem and hence give the information only with his consent. Then they are the counseling aspect. The client needs to understand that HIV/AIDS is not the end of an individual life and that the earlier he knows, the better for him.
Best Practices
To be effective in one’s responsibility as a case manager, there are certain practices that are essential. The first one is effective professional judgment. The case manager must have a structure and consistency in decision making. The case manager must be able to gather and organize specific information relevant to the case and the client. The information is then used to make the right judgment on what is necessary for the client. For example, the case of Jeff, getting all the information about him and the environment is key in making decisions affecting him. The second practice is the reliability in assessment of needs and risks. According to Douglas et al., (2016), a good case manager must have the information necessary to manage the client based on his needs and the risk factors involved. The case manager needs to have an understanding of protective factors. These are factors that reduce the chances of a continued negative behavior. An example can be family involvement. Like the case of Jeff, there is a big risk of him losing faith in the counseling process and may view it as betrayal if the information is not handled properly. The case manager or the service provider also needs to practice consistency. This involves the development of clear and consistent procedures and policies. The monitoring process needs to be defined.
Concept of Helping/Intervention Practices that Need to be Used in this Case
One of the intervention practices applicable to this case is counseling. Jeff needs a lot of counseling. At the moment, he does not understand himself. He is worried about so many things, and all he needs is comforting counseling. He needs to understand himself and understand that what he is going through is normal and will surely pass. The other helping practice applicable in development of individualized service plan. This will help track his progress during the counseling therapy and also define a clear monitoring procedure. This is the best way to keep him in check. Finally, the evidence based practice can also be applied. According to Epperson et al., (2014), the counselor combines his counseling expertise with the evidence that exists eternally to help Jeff. This involves giving him examples of individuals who have undergone the same ordeal and survived.
