INSTRUCTIONS:
1) Construct the fault tree, which should consist of:
a. both AND and OR gates,
b. at least 4 levels (the level is the maximum number of gates passed from a basic
event to the top event),
c. at least 9 basic / undeveloped events (undeveloped events should be justified).
2) Quantify the FTA.
a. Assign likelihood values to basic and undeveloped events with justification or
references.
b. Calculate the probability of the top event, based on the likelihood values assigned to
the basic and undeveloped events.
3) Discuss what prevention and control measures could be used to reduce the risks of a similar
