For section 10, these are the instructions below:
On the PMP template describe the change control process that will be used in your project. Include who approves the change and how the change will be evaluated. Include any diagrams that help explain how a change is requested, evaluated, approved, and notification of decision.
Also submit a 1 page Change control form that you will use for your project.
