Chronic disease is any condition that lasts a year or more and it requires ongoing medical care or it limits the patient's ADLs or both. Some of the chronic diseases that are the leading cause of death in the United States are cancer, heart disease, and cancer.
I live in Savannah, Ga. In my community, the two most prevalent chronic disease is diabetes and COPD. 23.7% of residents are diagnosed with diabetes. According to the CDC, more than 34 million people have diabetes in the US. There are people who don’t even know that they have. Having diabetes can increase a person’s risk of having a stroke or heart disease or other serious complications. The second chronic disease is COPD. 11 percent of savannah residents have COPD. Almost 15.7% of Americans have been diagnosed with COPD. COPD is the third leading cause of death for women.

Michigan's communities differ widely. There is a large portion of Michigan that is well-educated, generally healthy, and economically doing well. However, there are areas that live in poverty, In this research i have found areas with high racial segregation within the state where morbidity and mortality are substantially higher than the state average. For example, an analysis of 2010 Census data found that the Flint metropolitan area has a black-white dissimilarity index of 67, making it the 14th most segregated city in the nation. Within the state of Michigan (MI), Genesee County (the county that contains Flint) is ranked 81st of 82 counties for health outcomes in the Robert Wood Johnson Foundation County Rankings, and ranked 76th of 82 for health factors. Chronic lung disease and heart disease are the two biggest issues in my population at tis time.
