Assignment Case Study
COURSE SELECTED: HI253 Medical Coding I
Review the course outcomes included with the course.
Course Outcomes
*Identified appropriate diagnosis codes according to current guidelines.
*Recognized the information documented within a health record that leads to quality coding practices.
* Identified appropriate procedure codes according to current guidelines.
* Summarized the features of health information systems used for quality coding practices. *Evaluated health records for deficiencies needed for quality coding
Draw from your experiential learning and create a case study to demonstrate your understanding of/competency of one or more of the course outcomes. Incorporate real or hypothetical examples and integrate course appropriate terminology.
Experiential Learning
Studied Medical Coder for two years 2011-2013 on The English Center.
Employed as a full time Medical Coder for 5 years
Earned enough experience and I was promoted to Medical Auditor.
Coded 25 to 30 Primary Care Physician Clinical Notes daily, under CMS guidelines.
Conducted 30 patients’ prospective reviews to improve provider documentation.
Abstracted new Medicare Risk Adjustment codes from patient’s charts in order to code by Hierarchical Condition Category (HCC).
Calculated Organization patient’s panel risk scores.
Applied appropriate ICD 10 CM on mapping diagnosis on patient’s charts.
Applied appropriate CPT Codes on mapping procedures on patient’s charts.
Applied appropriate HCPCS Codes on mapping supplies, injections on patient’s charts.
Instructed and Monitored 4 Primary Care Physicians monthly to improve documentation.
Evaluated Electronic Medical Records according to current CMS Coding Guidelines.
Maintained and managed Hierarchical Condition Category (HCC) reporting process twice per year.
Certified Professional Coder.
For example in the course CE420 Curriculum Development, one of the course outcomes is to “Develop unit plans, curriculum lessons, and activities that support quality learning experiences for young children.” To demonstrate mastery of the outcome, you might discuss the strategies for developing, executing, and assessing an age appropriate lesson plan. The plan would address early childhood developmental stages, incorporate age appropriate multi-sensory activities, connect theory with practice by aligning with theories such as Jean Piaget’s Cognitive Theory, and include a post execution analysis.
Provide enough information for the evaluator to understand what you learned and how you used it. Connect the case study to a source of learning (such as a current or former position) so that the evaluator can track back to your expanded resume or to your learning autobiography to get more information if they need to.
Preparing Your Case Study
As you begin your case study, follow these guidelines to help you prepare and understand the case study:
1. Read and examine the course outcomes
· Review the course description to help you unpack the course outcomes
· Consider level of learning according to Bloom’s Taxonomy
2. Focus your analysis on one or more course outcomes
· Identify two or three competencies to validate
· What real life experiences do you have to demonstrate mastery of the concepts?
· How can you create a hypothetical example if a real life example is not feasible?
3. Link Theory with Practice
· Conduct outside research, consider your experiential learning, link theory with practice
· Integrate discipline specific terminology
· Demonstrate problem solving ability
· Show depth and breadth of knowledge
4. Select a Strong Example Case
· Incorporate supporting evidence, pros and cons, realistic solution
· Use real life examples to explain how the college‐level learning was applied
5. Consider Evaluation Criteria used by Portfolio/Course Petition Evaluators
6. Demonstrate college-level writing
· Write a five paragraph case study
· Include an introduction that summarizes the course description, clearly states the outcomes you are validating, and provides a declarative statement about your topic and purpose of the case study.
· Demonstrate critical and reflective thinking
· Use APA format
