The Verbatim Content (Narrative) is already completed and the Field Supervisor’s Comments section you do not have to do. The other sections: Automatic Thoughts, Feelings or Biases, Observations and analytical thoughts during an interview, What core social work skills, and Relevant Competencies and Practice Behaviors sections has to filled in. You do not have to put something for each box but there needs to be a good amount of them filled in. The Relevant Competencies and Practice Behaviors section are referring to the Social Work competencies and practice behaviors and can be looked up by doing a google search. I have filled in some here and there on the other sections as an example.
SW 691 Process Recording Intern: Laquita Robinson Field Placement: VA Clinic-JXN Field Instructor: Patricia Cunningham Date: March 26, 2019 Identifying Information: B, is a 62-year-old, independent Black female. She is a homeless veteran, who has over the past 6 months lived in and out of motels and in her car. She appeared to be hopeful and optimistic about starting online courses and sustaining permanent housing. She appeared to be clean, neat, and professionally dressed. Purpose of the session: The initial assessment to assist the client in establishing a plan to sustain permanent housing and to assess for other helpful resources and/or services, through techniques of asking relevant open-ended questions and active listening. Initial Observations: I met with B in the office at the VA Clinic. She appeared to be full of life and eager to get the process started for housing. She was very open about sharing personal information with me. After asking questions and providing B with a summary of what the program was about, the client was ready to work together.
